There are a few options to get started on the registration process.

First, click on this link and fill out the form.

Next, click here to print the registration packet (single sided) at home. Click on EACH tab and print out EACH form. You may also pick up a packet at the EOE office or pick one up at the Tumwater District office.

If your child was born in Washington, you may print out their immunization record here. Please bring that copy with your registration packet. 

Completed forms may be dropped off at the EOE office on or after.
