Taking an Extended Leave
General Leave Information

For any extended leave of five days or more, please complete a Leave of Absence Request form.
If you wish to apply for Shared Leave, please complete a Shared Leave Application and send it to Human Resources.
Please contact your HR Rep for more information about leave options.

Federal Family & Medical Leave (FMLA)
To determine eligibility for FMLA, HR will need the following forms completed and turned in:
- Leave of Absence
- Certification from Health Care Provider:
- For Yourself
- For a Family Member

Washington Paid Family Medical Leave (PFML)
If you may qualify for Paid Family Medical Leave (PFML), here are some helpful resources:
- Please turn in a complete Leave of Absence form to HR and communicate your intent to apply for PFML. We encourage you to notify your supervisor and HR at least 30 days in advance, unless there are extenuating circumstances that prevent you to provide notice this early.
