• Health Records

Health Services protects the confidentiality of your child's health records (TSD Board Policy 5600; Administrative Procedure P-5600.)

In general, medical information cannot be disclosed without parent/student consent. Exceptions are allowed on a 'need to know' basis for student safety (RCW 70.02.050). In order for district Health Services to request or release additional health records, a parent/guardian/student must sign the Authorization for Mutual Exchange of Medical/Health Information.

The Family Educational Rights and Privacy Act (FERPA) gives parents and eligible students over 18 years of age certain rights with respect to the students health and educational records.

Immunization records are available to graduating seniors as they may need a copy of their Certificate Of Immunization Status (CIS) for employment, college entrance, or military enlistment.
