• Emergency Information

    Each school year parents/guardians must complete and sign an Emergency Information form. This form provides an annual update of your child's health status and gives the school permission to authorize the transport and provide emergency treatment for your child should the need arise.

    Please remember to notify the school for any changes in the names and/or phone numbers of the emergency contacts for your child.

    Life Threatening Conditions
    Washington State Law requires schools to have in place emergency care plans for any student with a "potentially life-threatening health condition" before the child begins attending school. This may include, but is not limited to, children with severe allergies, severe asthma, diabetes, or a severe seizure disorder. For more information related to life threatening health conditions, please contact the school nurse at your child's school (Health Services Schedule).
